Please complete the complaint form. Upon submission, the complaint will be submitted to the Board of Nursing.
1.The complaint process must be completed in one sitting/session.
2.You cannot save and return later.
3.The complaint process will time out after 30 minutes of inactivity.

Before you begin, you may want to collect these pieces of information: nurse’s license number (if known) and contact information; any patient and witness information that is relevant; and date, location, and description of the incident. You will also have the opportunity to upload supporting documents if you wish.
